When care is needed quickly, after a hospital stay, a sudden change or when a family carer can no longer cope, call us and we will tell you honestly what we can do.

Care is subject to assessment and availability.

If someone is seriously ill or injured, call 999. For urgent medical advice, call NHS 111. We provide care and support at home, not emergency medical help.

When You Might Need Care Quickly

  • Coming home from hospital

Support to settle back in safely once a discharge date is set.

  • A sudden change

A fall, an illness or a loss of confidence that makes everyday tasks harder overnight.

  • A family carer can no longer cope

When the person who usually helps is unwell, exhausted or called away.

  • Care has fallen through

When existing arrangements end unexpectedly and you need a new plan.

What Happens When You Call

  1. You call us

Tell us what has happened, where the person lives and what support is needed.

  2. We check availability

We are honest straight away about whether we can help and when.

  3. A quick assessment

We arrange an assessment, at home or in hospital where possible, to understand needs and keep everyone safe.

  4. Care begins

Once the assessment is complete and carers are confirmed, we agree a start time and a simple first care plan.

  5. We review

In the first days and weeks we check in often and adjust support as recovery or needs change.

Planning Care for Coming Home From Hospital

The best time to call is as soon as you know a discharge is likely, not on the day itself. That gives us time to assess, plan visits and make sure someone is there when the person arrives home.

It helps to have ready:

  • The expected discharge date, if known
  • The address and postcode where care is needed
  • A short summary of what help is needed with washing, dressing, meals and getting about
  • Contact details for a family member or the ward, if appropriate

With your permission, we can talk to family members and the professionals involved so everyone is working from the same plan.

Support We Can Put in Place

  • Settling in

Being there when someone arrives home, warming the house and making a drink and a meal.

  • Personal care

Help with washing, dressing and using the toilet while strength returns.

  • Medication support

Prompts and reminders in line with the care plan.

  • Meals and shopping

Making sure the fridge is stocked and meals are not missed.

  • Safety at home

Moving safely between rooms and noticing anything that needs attention.

  • Short-term or ongoing

Support for a few weeks while you recover, or longer if needs change.

Our Honest Approach

  • Clear answers

We will tell you plainly if we can help and when. If we cannot, we will say so quickly.

  • Safety first

We only start care once we have assessed needs and can staff visits properly.

  • Families involved

We keep relatives updated, with consent, so nobody is left guessing.

  • Ready to adapt

Short-notice care often changes fast. We review support as often as needed.

How Much Does Short-notice Care Cost?

Short-notice care is charged in the same way as our visiting and live-in care, based on visit length, frequency and time of day. We will explain costs clearly before care begins. If you are leaving hospital, ask the ward about any council-funded support you may be entitled to.
